I had to deal with something similar a few years ago. Submitted my "Registration of birth abroad" for my passport and was denied because it was a photocopy. The same one they had returned to me 5 years earlier when previously renewing my passport.

I had to apply for a Citizenship Card even though I have always been a Canadian citizen complete with a DND birth certificate and 20 years in the military.

But that pales beside what some of these folks have to deal with.
