BC is an exception, and you only pay if you don't file a claim you made less than $28,000 net the previous year.
It's disgusting we're among the last Canadians to pay, the premium was ruled illegal back in the VanderZalm days. WAC Bennett instituted the PST as a 'Social Service Tax" and that was earmarked originally for medicare and Hospital funding.
2 things I want to see happen before I pass on. No medical premiums and no provincial sales or purchase tax on your FIRST primary residence.
EDIT: In light of the last post, I'd like to point out that the one thing I despise about American noe-cons dissing our system is their concept that money somehow entitles you to better medical service than any other citizen. Disgusting, selfish, and all too commonly the biggest argument for keeping the status quo. The second biggest BS is that a UHC plan delivers lower quality services.
