Because the former happens all the time. Usually with the severely disabled or comatose patients. They get stool hardeners one day, so they are constipated, then laxatives the next and they get to sit for a few hours on the toilet while the overworked and underpaid TFW "nurses" attend other people with a severe case of the poops.
This lady might just be the latest example, but she is far from unique. I'm more for the latter definition of Ceausescu'd, in that case.
There will always be those who take advantage of any system but in ours if you see a story inviting justified outrage such as the one in the OP you can expect something be done.
There was a time when one considered the responsibility of family a priority. It was a better time. Family will offer care for each other in a way governments won't so the idea a 94 year old family member should be close enough to a family member that they can watch over her situation was taken for granted.
At one time, not just at Christmas but especially at Christmas administrators of an old folks home would not dare lock a 94 year old in a bed-bug infested room to be a "blood buffet." They'd have to worry about a son or a daughter walking in and after being sufficiently horrified complaining to authorities.
In the system PD would like us living under those authorities would be more likely to send an "official" or "officials" to visit the son or daughter if they dared complain.

I meant more, a hasty show trial followed quickly by dragging the perpetrators out back, tying them up, lining them up to a wall, and letting the volunteers fire until dead. then footage of the justice being played on national television
The advantage of our system over the system operating under the ideology preferred by PD (think China to North Korea) is you can complain about outrages such as the one described in the OP without having to worry about getting the 3 o'clock visit from the secret police.
